  • We accept cash, checks, Visa, MasterCard, Discover and American Express.
  • Itemized receipts given to patients are accepted by most insurance companies for direct reimbursement when attached to your claim.
  • See Insurance for contracted Insurance Plans list. List is updated frequently. Call the office regarding unlisted plans that may prohibit out of network services. Insurance cards are required at time of service for contracted plans. Accurate claims address, insured and policy holder’s identification and group numbers must appear. Missing information prohibits processing. Patients are responsible for amounts due.
  • Payment for office visits, laboratory services, or in-office diagnostic procedures are expected at the time of service. Fees are clearly marked on itemized receipts. Do not hesitate to ask for further explanation.

Your insurance plan may require you to pre-certify certain services or to notify them within a certain period of time after hospitalization. You should:

  • Read the details of your plan’s requirements and any benefit documents provided by your insurance carrier or employer.
  • Talk to your doctor’s office and your insurance company about elective procedures.
  • Discuss any insurance eligibility or payment concerns when you schedule your appointment or ask to speak with a financial counselor.

Insurance Information

Your health is our top priority. That’s why we accept a wide range of insurance providers—because you should never have to worry about a change in coverage affecting your well-being. Some health plans require patients to receive services at an “in-network” or “participating” provider. Contact your insurance to verify Northwest Community Healthcare is an in-network provider.
The Amount You Owe

We will bill your insurance plan(s) on your behalf. After that, you’ll receive an “Explanation of Benefits” (EOB) from your provider. This will show the amount they have paid, any non-covered or denied amounts and the remaining balance that you owe.

  • Review this information and call your insurance company or NCH if you have questions.
  • NCH will send you a bill for any remaining amount due (such as co-insurance, deductible or non-covered charges).
  • You may be asked to pay at registration or discharge from the hospital.
Out-Of-Network
  • In an emergency, you should go to the nearest hospital, even if it is “out-of-network.”
  • Your insurance plan will generally cover these costs or transfer you to an “in-network” hospital.
  • If you go to an “out-of-network” hospital in a non-emergency situation, you may be required to pay a larger portion of your bill.
  • Contact your insurance company for details about its “out-of-network” coverage.
